Proof of eligibility to work in the U.S. (unrestricted Social Security card, U.S. passport, Permanent Resident card, Employment Authorization, U.S. birth certificate, foreign passport with appropriate visa, etc) and ID

Annual physical
- Annual TB screening (negative ppd, or negative chest x-ray in case of positive ppd)
- Lab test results and / or immunization records showing immunity to:
Hepatitis B, Rubeola, Rubella, Varicella - Drug screening
- Two recent professional references
- Competency testing
- Annual Core Mandatory testing
For RN's and LPN's:
- Current signed nursing registration license in NYS
- Current malpractice insurance in the amount of $1/3 million
- BLS
- Evidence of completion of nursing education - diploma or transcript.
For HHA's/PCA's:
- Original HHA/PCA certificate by approved DOH/DOE training program
- CHRC fingerprint requirement
