Private Duty Nursing
Private Duty Nursing services (LPN's & RN's) are available for patients of
all
ages who require personalized one-on-one care. These nurses are
direct care
providers who are paid on an hourly basis. Their skill levels and expertise vary according to their specialties. Each nurse works in collaboration with the
patient's primary care physician and is knowledgeable in the administration
of medications, checking blood sugar levels, providing wound care, suctioning,
providing tracheotomy care, giving injections, monitoring changes in medical
status, as well as following prescribed treatment protocols.
Visiting Nursing upon Request/As Needed
Registered Nurses are available to visit any patient who may request a
personalized health consultation. Our licensed professionals are highly trained
and technically proficient to render care, treatment and services.
Care plans are developed with input from the patient, caregiver,
and/or family members, and we work closely with the primary physician and
other members of the health care team. Services may include but are not limited to:

Medical care planning with physicians and hospitals

Medication management

Wound care

Diabetic teaching

Nutritional instructions

Assessment and diet teaching
Respite Care
We can
arrange for temporary or short-term relief (respite care) for the primary caregivers and family members who are responsible for caring for loved ones at home. These loved ones might be frail, functionally impaired, or
disabled, have debilitating illnesses, and/or require long term care. Our
goal is to provide intervention on an intermittent level to alleviate stress
and overwhelming conditions that may be encountered by caretakers when
dealing with prolonged illness. We will develop a plan of care to facilitate
the appropriate treatment and services desired and work collaboratively with
all members of the health care team to achieve optimum results.
Live In Services
Live in services are ideal for individuals with limited mobility who may be prone to falling. In addition, patients suffering from Alzheimer's or
dementia or recovering from a debilitating illness, or those whose close
family members live far away, may derive great benefit from this type of
service.
We will work with caregivers and family members to arrange for 24-hour live
in services in the patient's home. Our carefully screened aides are highly
trained to provide the optimum level of care in the home. Our rates are
reasonable, and around the clock service offers many benefits to the
patients and loved ones.
Companions
Our companion services are available upon request for patients who may need
to be accompanied to their medical appointments, social gatherings, daily
walks, or religious services. Staff will assist with light housekeeping,
meal preparation, and shopping, in addition to building rapport by
interacting and conversing with the patient. This may involve reading,
playing games (cards, scrabble, etc.), or other exercises. The goal is to
assist the client in maintaining an optimal level of independence at home.
Personal Care Services (HHA/PCA)
Our personal care services help individuals who may be elderly, disabled,
recovering, or chronically or terminally ill, who are in need of assistance
with their activities of daily living and/or other support services. These
individuals have elected to receive non-institutional care and remain at
home. Depending on the level of care/treatment/services needed, our home
health aides or personal care aides are available to provide the appropriate
level of support and assistance.
Services provided include but are not limited to:
Personal care, grooming, assisting with activities of daily living, mobility
assistance, meal preparation, cooking, accompanying to medical appointments,
light housekeeping/cleaning, laundry, grocery shopping, errands, etc.
Housekeeping
Our aides are available to performs housekeeping duties such as:

Cleaning up bathroom and kitchen after use
